{"id":104595,"date":"2020-12-30T23:29:20","date_gmt":"2020-12-31T04:29:20","guid":{"rendered":"http:\/\/www.thevalleyledger.com\/?p=104595"},"modified":"2020-12-30T23:30:16","modified_gmt":"2020-12-31T04:30:16","slug":"allentown-health-bureau-scheduling-covid-vaccinations-2-2","status":"publish","type":"post","link":"https:\/\/www.thevalleyledger.com\/?p=104595","title":{"rendered":"ALLENTOWN HEALTH BUREAU SCHEDULING COVID VACCINATIONS BY SURVEY MONKEY"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p>The Allentown Health Bureau is now utilizing Survey Monkey to schedule appointments for the Moderna COVID-19 vaccine for qualifying healthcare staff in Lehigh County.\u00a0This will allow those qualifying persons to book their own appointments.<\/p>\n<p>In accordance with regulations established by the CDC, appointments for COVID vaccination can be scheduled for members of the following groups ONLY:<\/p>\n<ul>\n<li>Licensed medical professionals &amp; care staff including but not limited to: EMS professionals, dentists, podiatrists, dermatologists, ophthalmologists, primary care physicians, etc.<\/li>\n<li>Home health agency staff including hospice, visiting nurses, physical therapy, occupational health, home health aides, etc.<\/li>\n<\/ul>\n<p>Please follow the link below to schedule your appointment for the COVID-19 vaccine.\u00a0 Once the forms are completed in Survey Monkey, you will be guided to PREPMOD to schedule your appointment.<\/p>\n<ul>\n<li><a href=\"https:\/\/www.surveymonkey.com\/r\/HCW_AHB2020\">https:\/\/www.surveymonkey.com\/r\/HCW_AHB2020<\/a><\/li>\n<li>Upon scheduling your appointment, you will receive a confirmation email.\u00a0 Please allow up to 24 hours to receive your confirmation email.\n<p>The Survey Monkey link works best in Chrome.
